NDIS Specialist Support Coordination

Specialist support for complex needs

Specialist Support Coordination is like a dedicated team that helps improve your NDIS plan.

They assist with understanding the NDIS, integrating into the community, and ensuring you receive the necessary support. If you have Specialist Support Coordination, you can also access expertise from professionals like Occupational Therapists, Psychologists, or Social Workers to optimize your support and plan. This aims to simplify and reduce complexities in your situation over time, promoting independence.

If your needs change, the NDIA might recommend regular Support Coordination instead of Specialist Support Coordination as your plan progresses.

Frequently asked questions

If you’re eligible for NDIS funding, you might receive specialist support coordination based on your disability and individual needs. If you don’t have a NDIS plan or specialist support coordination in your plan, you can talk to our friendly staff to explore your options.

Breakthru has many well-trained and experienced specialist support coordinators located in New South Wales, Queensland, and Victoria. You can find a specialist support coordinator near you by checking out our locations or simply giving us a call at 1800 767 212.

Specialist Support Coordination comes into play when you need a specialist like an Occupational Therapist, Psychologist, or Social Worker to provide Support Coordination-style activities.

If your disability, injury or health condition is complex or if you have multiple diagnoses, it is more likely that you can access a specialist support coordinator.

For anyone with a disability, injury or illness that is less complex or with fewer diagnoses, a support coordinator may be more appropriate. These decisions are made by your Local Area Coordinator when assigning your NDIS plan.

Specialist Support Coordinators will also cover the supports provided by Support Coordinators, but depending on your situation and individual needs you may have access to both.

Get started in 4 easy steps

If you have a significant and permanent disability, injury or illness that affects your daily life, you may be eligible to access the NDIS.

Step 1

Check your eligibility

Before you begin, use the NDIS checklist to confirm that you:
  • are aged between 7 and 65.
  • reside in Australia with Australian residency.
  • require immediate support to lessen future needs.
Step 2

Make an Access Request

To start your application, call the NDIS at 1800 800 110.

If you need assistance, reach out to your Local Area Coordinator, Early Childhood Early Intervention partner, the local National Disability Insurance Agency, or Breakthru for support.

Step 3

Gather Supporting Evidence

This may involve sharing details about your disability and how it affects your daily life.

Providing recent information and collaborating with your healthcare providers can strengthen your case for NDIS eligibility.

Step 4

Prepare for your planning meeting

Once the Access Request decision is done, you'll receive a written notice of the decision.

Afterward, you'll meet with an NDIS community partner to determine the support level for your NDIS plan.
