Support Coordination for Regional & Remote Communities

With over 30 years of experience operating in regional, rural and remote communities across Australia, we understand the unique needs and challenges faced by people with disability.

With help from our experienced Support Coordinators, we’ve designed a flexible and fully remote support model to ensure you can access the services you need and get the most out of your plan – no matter where you are in Queensland, New South Wales, or Victoria.

How we support you

Our Support Coordinators are members of the Breakthru Communities of Practice, which is a group of 100+ Breakthru Support Coordinators dedicated to sharing information, staying up to date on NDIS changes, and continuing professional development. This ensures our Support Coordinators are continually developing their skills so they can provide the best possible support to you.

We'll assist you in understanding your NDIS plan, including how you can use specific types of funding, processes you need to follow, and ways to better connect with the NDIA in a regional or remote setting.

We understand options can be limited in regional and remote areas. Our experienced Support Cooridnators will work with you to find suitable providers and give you creative solutions to ensure you can access the services you need.
Report and Goal Tracking

We'll track progress toward your goals, so you can see how much you've achieved with your NDIS plan. We'll also report back to the NDIA, so they can be sure your plan is meeting you needs, or provide additional funding should you need it.

Travel, internet access and poor reception are common barriers in regional and remote communities. Our Support Coordinators will take the hassle out of booking services and communicate with you to ensure you have everything you need.

We'll work closely with you and your nominated plan manager or NDIA representative to ensure you're using your budgets efficiently and getting value for money when it comes to supports through your NDIS plan.

Our Support Coordinators are based in regional and remote communities across Queensland, New South Wales, and Victoria, including areas such as:

  • Buxton, NSW

  • Dubbo, NSW
  • Tullamore, NSW

We believe having this local knowledge about service providers and the unique challenges faced in these areas provides our clients with greater choice, control, and higher quality support.

Funding in your NDIS plan is based on what the NDIA considers to be ‘reasonable and necessary’ to achieve your goals, with consideration to the support you may already receive from friends, family or other Government agencies.

If you do not have Support Coordination in your NDIS plan, you should speak with your NDIA planner at your next planning meeting and ask for this to be included in our Capacity Building budget.

Support Coordination is a type of Capacity Building support designed to help you best utilise your NDIS plan.

Support Coordination is designed to:

  • Understand your NDIS plan to pursue your goals
  • Connect with NDIS providers, mainstream and other Government services
  • Build your confidence and skills to self-manage and coordinate your own supports.

 

Get started in 4 easy steps

If you have a significant and permanent disability, injury or illness that affects your daily life, you may be eligible to access the NDIS.

Step 1

Check your eligibility

Before you begin, use the NDIS checklist to confirm that you:
  • are aged between 7 and 65.
  • reside in Australia with Australian residency.
  • require immediate support to lessen future needs.
Step 2

Make an Access Request

To start your application, call the NDIS at 1800 800 110.

If you need assistance, reach out to your Local Area Coordinator, Early Childhood Early Intervention partner, the local National Disability Insurance Agency, or Breakthru for support.

Step 3

Gather Supporting Evidence

This may involve sharing details about your disability and how it affects your daily life.

Providing recent information and collaborating with your healthcare providers can strengthen your case for NDIS eligibility.

Step 4

Prepare for your planning meeting

Once the Access Request decision is done, you'll receive a written notice of the decision.

Afterward, you'll meet with an NDIS community partner to determine the support level for your NDIS plan.
