About Our Sydney Support Coordinators   

When you work with Breakthru, you’ll connect with a trusted Support Coordinator that will help you navigate the NDIS.  All of our Support Coordinators also have access to our National Community of Practice, which is a network of all 70+ Breakthru Support Coordinators nationally where they share information and advice.

This allows us to stay up to date with changes from the NDIS, identify providers with the shortest waitlists and quickly respond to crisis situations with the help of a peer support network. All of this helps support you and your goals, as it ensures your Support Coordinator remains an expert in their field and someone that can give you strong advice.

You may be eligible to receive NDIS funding for Support Coordination. Factors that could impact your eligibility include your personal circumstances, such as your disability and individual needs. While a support coordinator will help navigate your NDIS plan and find high-quality disability service providers, a psychosocial recovery coach will provide capacity-building support. They will not provide core support for daily activities or community, social, and recreational activities.   

Psychosocial recovery coach services are tailored to the needs of individuals with primary psychosocial disabilities, focussing on coaching and collaboration with other services. 

You may choose to have both a support coordinator and a psychosocial recovery coach, depending on your circumstances and your NDIS plan.

Get started in 4 easy steps

If you have a significant and permanent disability, injury or illness that affects your daily life, you may be eligible to access the NDIS.

Step 1

Check your eligibility

Before you begin, use the NDIS checklist to confirm that you:
  • are aged between 7 and 65.
  • reside in Australia with Australian residency.
  • require immediate support to lessen future needs.
Step 2

Make an Access Request

To start your application, call the NDIS at 1800 800 110.

If you need assistance, reach out to your Local Area Coordinator, Early Childhood Early Intervention partner, the local National Disability Insurance Agency, or Breakthru for support.

Step 3

Gather Supporting Evidence

This may involve sharing details about your disability and how it affects your daily life.

Providing recent information and collaborating with your healthcare providers can strengthen your case for NDIS eligibility.

Step 4

Prepare for your planning meeting

Once the Access Request decision is done, you'll receive a written notice of the decision.

Afterward, you'll meet with an NDIS community partner to determine the support level for your NDIS plan.
