About the role 

As a Support Coordinator, you will support our customers to achieve their personal goals and to live satisfying lives in our community. Our approach is simple, we focus on the customer’s choice of supports to achieve their individual goals and ensure they are delivered where the customer prefers.

Please note as referrals increase there is an opportunity to increase your work hours as well.

Key responsibilities include:

  • Supporting our customers to achieve their goals
  • Maintain and establish links to community
  • Manage customer plans and supports
  • Meet set KPI’s
  • Promoting independent decision making
  • Work within the principles of person-centred practice

About you 

You are a passionate and dedicated individual who is looking to make a difference in the community. As a people person, you pride yourself on exceptional customer service whilst ensuring customers achieve their goals and aspirations.

You will also be able to clearly articulate and demonstrate:

  • Mental Health, Disability or Community Services qualification or relevant experience (desirable)
  • Ability to work in a team and individually
  • Ability to build rapport with a diverse range of customers
  • Experience working with customers with complex support needs
  • Driven, energetic and committed
  • Strong communication skills
  • Current driver's licence (essential)
  • Working with Children check and current police check (essential or willing to obtain)
  • NDIS Worker Screen (essential or willing to obtain)
  • COVID-19 Vaccination Certificate or provide a medical exemption evidenced by an authorised medical practitioner
