EMOTIONAL WELLBEING

At breakthru we provide support to people to assist them with their mental health concerns to maintain the best possible social and emotional outcomes, and meet personal recovery goals.

breakthru can work with you one-on-one or in partnership with carers, families and other health professionals, to develop and implement a recovery plan tailored to meet specific preferences.

Depending on needs, breakthru can assist with a referral to:

Community support

Outreach to your home to support you to achieve goals around recovery with your mental health. This can include group support.

Respite for carers

Support to people who care for somebody with a mental health issue or autism. This can include group support.

Prevention and recovery care

A voluntary, short term residential facility to assist in times of relapse of a mental health condition.

Peer support

breakthru has employed people who have a lived experience in mental health and wish to share their recovery journey to instill hope.

Family support

Support to children and caregivers when mental health issues are emerging in children or behaviours are becoming a concern.

Homelessness assistance

Outreach support to people who are homeless or at risk of homelessness who have been admitted to a psychiatric facility.
